At your first visit to Brazos Oral & Facial Surgery, you will be greeted by our friendly office staff who collect the proper registration information. We ask that you please assist us by providing the following information at the time of your consultation:
- Your referral slip from your dentist.
- Any applicable X-rays and list of medications you are presently taking.
- To protect against identity theft, we ask that you bring a driver’s license or other government-issued photo ID.
To make the patient registration process run as smoothly as possible, you complete and submit the registration form online prior to your initial consultation.
During your initial appointment, Dr. Warren will perform a consultation, review your symptoms, explain your diagnosis, and outline his recommended treatment options. Our staff will help you determine the treatment plan that you feel comfortable with. We will also explain your anesthetic options so you can choose the best option for anesthesia or sedation during the procedure.
In some circumstances, surgery can be done the same day as the consultation, but a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day. It is required that a parent or guardian accompany you to the consultation visit if you are under 18 years of age.
Please alert the office if you have a medical condition that may be of concern prior to surgery. This includes conditions such as diabetes, high blood pressure, artificial heart valves and joints, or rheumatic fever. Please notify our office if you are on any medication like heart medications, aspirin, or anticoagulant therapy.
